Patient, Client, Resident, Family and Visitor’s Rights
At Providence Care, you have the right to expect that all personnel will respect and promote your rights:
- To know who the members of your care team are responsible for your treatment.
- To access information that Providence Care holds about you in accordance with the law and Providence Care’s policies.
- To actively participate in assessing your needs and developing or updating your care plan.
- To choose a person to be present during assessments or planning discussions.
- To receive help coordinating your services and care.
- To give or refuse consent for any care or service, unless there is an exception to that consent under the law.
- To ask your care provider if they have washed their hands.
- To be treated with respect and be free from all forms of abuse and discrimination.
- To have your dignity, privacy, choices and independence respected.
- To receive care and services that recognize your individuality and respond to your needs, culture, language, faith, gender expression, sexual orientation, and family preferences.
- To share feedback, raise concerns, or suggest improvements without fear of retaliation, discrimination or judgment.
- To be informed about how to make a complaint and how it will be addressed.
- To receive care without discrimination, as protected by the Human Rights Code and the Canadian Charter of Rights and Freedoms.
- To have your personal health information collected, used and disclosed in accordance with the law and Providence Care’s policies.
Patient, Client, Resident, Family and Visitor Responsibilities
As a patient, client, resident, family member, or visitor, you are expected to:
- Treat all people, including Providence Care personnel, patients, clients, residents, families, and visitors with courtesy and respect. Harassment, racism, discrimination, and verbal and physical violence are not tolerated.
- Contribute to a safe environment for everyone.
- Provide accurate, timely and complete information about your health, medications and support to your care team.
- To actively participate in developing your care plan to the best of your ability.
- Follow your care plan to the best of your ability and ask questions when needed.
- Inform your care team about any changes to your health or available support.
- Understand that others’ needs may sometimes be more urgent than your own.
- Keep track of and take responsibility for your personal belongings.
- Work with personnel to address and resolve any concerns about your care.
- Support development of your treatment and care plan by sharing relevant information about your lifestyle, including substance use, smoking, diet, or exercise, even if it’s difficult to discuss.
- Respect the privacy and confidentiality of other patients, clients, residents, families, personnel and visitors.
- Participate in and prepare for discharge early by ensuring you understand your instructions, medications, and follow-up care.
Support safety measures such as wearing your identification band, following infection control practices, which can include  washing your hands and wearing appropriate personal protective equipment.
