Fundraising Notice
As part of its fundraising efforts, the University Hospitals Kingston Foundation (UHKF) reaches out to patients, residents, and clients to invite them to consider making a donation to Providence Care.
Providence Care relies on the generosity of its donors to help us deliver the best care and provide the best opportunities in our hospital, home, and community programs for the benefit of everyone in our community. As part of our fundraising efforts, our Foundation, University Hospitals Kingston Foundation (UHKF), contacts patients, residents and clients of Providence Care to request that they consider making a donation to Providence Care.
Unless you request otherwise, your implied consent will allow:
- Providence Care to provide your name and mailing address (and the name and mailing address of your substitute decision-maker, where applicable) to UHKF; and
- UHKF to use this information to contact you to request a donation.
You may withdraw your consent for the use of your information for these purposes by contacting the Privacy Office and Freedom of Information Office at:
