Assertive Community Treatment Program
The Assertive Community Treatment (ACT) program support individuals living with serious and persistent mental illness. The program provides 24/7, recovery-focused treatment, rehabilitation and support services. This service is designed for individuals who need more intensive support than traditional outpatient programs can provide.
Who We Help
This program supports individuals aged 18 and older who:
- Have a serious and persistent mental illness (e.g., schizophrenia, other psychotic disorders or bipolar disorder)
- Need for intensive support to live safely and successfully in the community
- Have not benefited from other community mental health services
What to Expect
This outpatient program provides:
Support from a multidisciplinary team including nurses, occupational therapists, social workers and psychiatry
24/7 care: The team is available to support you after-hours, weekends and holidays
Medication support, symptom management, and physical health care
Help with daily living, housing, and community integration
Support for achieving personal recovery goals
Family and care partner involvement your care.

How to Access the Service
For Patients and Care Partners:
Speak with your healthcare provider about a referral to our program.
For Referring Partners
Patients can be referred using the South Eastern Ontario Addictions & Mental Health Service Access referral form.
Eligibility criteria
- 18 years or older
- Have a primary diagnosis of a serious and persistent mental illness
- Require high levels of support in daily life and community living
- Have challenges accessing or benefiting from traditional outpatient care

Appointment & Discharge Information
Appointment
The care team is flexible and can meet you in the office or community.
Discharge
- Ask for explanations and written instructions about your care and your medications.
- Make sure you and/or your care partner understand the instructions, including what medications to take, how to take them, and when to take them.
- Ask and be informed of appointments that have to be made when you are discharged, how to contact the providers, and to understand the reason for each appointment and what information you will need to bring with you.
- Know who you should call if you have questions after discharge – write down contact names and phone numbers or have a member of the care team provide this information to you in writing.

Our facilities operate 24/7, but building access and available services vary by location and time of day. Some entrances may be locked, and ID badge or intercom access may be required.
