Forensic Mental Health Outreach
The Forensic Mental Health Outreach program supports adults who have come into conflict with the law and are living with a serious mental illness. The program provides specialized assessment, treatment, rehabilitation, and community reintegration services tailored to each client’s needs.
Who We Help
This program supports individuals 18 and older who:
- Have a serious mental illness
- Have come into contact with the law as a result of committing a crime
What to Expect
This outpatient program provides:
Personalized care plan:
Developed collaboratively by an interprofessional team including psychiatrists, nurses, social workers, and behavioural technologists.
Assessment and follow-up:
Comprehensive assessments are conducted, with individualized recommendations, treatment plans, and next steps provided.
Rehabilitation and reintegration support:
Assistance with skill development, social supports, and strategies for safe community living.
Family and care partner involvement:
Encouragement for care partners to participate in the care process when appropriate.

Referrals to additional mental health, legal, and social services as required.
How to Access the Service
For Referring Partners
Referrals can be made by:
- Primary Care Practitioners
- Defense lawyers
- Adult Mental Health Services (inpatient and community based)
Referrals can be made using the Forensic Mental Health Outreach referral form.
Eligibility criteria
- Age 18 or older
- Serious mental illness
- Involvement in the criminal justice system or concerns over potential entry

Appointment Information
What to Bring
- Your Ontario Health Card
- An up-to-date list of all medications you are taking using, including non-prescription, vitamin, and herbal preparations
- Any identified current/active court documents ie. bail conditions
You may bring and use cell phones and tablets; however, to protect the privacy of others, please do not take photos of people without first asking permission. When using a device, please use headphones.
You are encouraged not to bring valuables. Providence Care is not responsible for the loss, theft or damage of any personal belongings.

Hours of Operation
Our facilities operate 24/7, but building access and available services vary by location and time of day. Some entrances may be locked, and ID badge or intercom access may be required.
