Geriatric Medicine Clinic
The Geriatric Medicine Clinic supports older adults with complex health issues, including memory concerns, polypharmacy, balance impairment/falls, and multiple medical conditions. Our team works with you and your care partners to optimize independence, safety, and quality of life.
Who We Help
This program supports older individuals (most often aged 65 and older) who:
- Are at risk of health decline and have multiple chronic health conditions
- Are experiencing memory loss or cognitive decline
- Are at risk of losing independence
- Are dealing with functional decline, falls or mobility issues
- Need support with medication review/optimization or care planning

Our geriatrics specialized doctors work with you and your family to develop a care plan focused on your individual needs.
Comprehensive assessment:
A full assessment with a Geriatrician or Care of the Elderly physician (doctor who specializes in aging). You may also undergo cognitive testing with one of our specialized nurses.

How to Access the Service
For Patients and Care Partners:
Speak with your healthcare provider about a referral to our clinic.
For Referring Partners
Patients can be referred through the Geriatric Medicine Outpatient Referral Form.
Eligibility criteria
- 65 years of age or older (with some case-by-case exceptions)
- Have multiple co-morbidities or clinical frailty
- Live in the community
- Experiencing functional decline, gait and balance issues, falls, cognitive impairment, or polypharmacy

What to Bring
- Your Ontario Health Card
- Adaptive aids including walkers, wheelchairs, hearing aids, etc.
- A water bottle
- An up-to-date list of all medications you are taking using, including non-prescription, vitamin, and herbal preparations
You may bring and use cell phones and tablets; however, to protect the privacy of others, please do not take photos of people in the hospital without first asking permission. When using a device, please use headphones.

What to Expect
- Appointments take place in a quiet, private outpatient clinic at Providence Care Hospital. Report to Registration on level 1.
- After registering, go to the Clinics waiting area located to the right of the Welcome Desk. A staff member will take you to the assessment room.
- First visits usually take between 1 to 1.5 hours while follow-up visits are often more focused (approximately 30 minutes).
Discharge
After your appointment, a detailed report will be sent to your referring provider. We’ll coordinate any recommended follow-up care or community supports.
- Ask for explanations and written instructions about your care and your medications.
- Make sure you and/or your care partner understand the instructions, including what medications to take, how to take them, and when to take them.
- Know who you should call if you have questions – write down contact names and phone numbers.
