Hospice Care Coordination Program
The Hospice Care Coordination Program supports individuals with a progressive life-limiting illness at an advanced stage and their care partners. The Care Coordinator helps identify individual needs and goals, and assists with system navigation, referrals, information, and advocacy.
Who We Help
This program supports individuals aged 18 and older and their care partners who:
- Are living with a progressive life-limiting illness at an advanced stage
- Need help navigating hospice and community palliative care services
- Want support with planning, referrals, and access to volunteer resources
What to Expect
A Care Coordinator will work with you to understand your needs and wishes.
We help you:
Identify your goals of care
Navigate health and community services
Access trusted information and resources
Receive advocacy and support with referrals
Coordinate and monitor volunteer services that support you and your care partners
Support is flexible and based on your preferences.

No. We work alongside other health care professionals. Please note: This is not a 24-hour crisis support line.
Is there a cost to this program?
No. This program is provided at no cost, thanks to the generosity of community donors.

Hours of Operation
Our facilities operate 24/7, but building access and available services vary by location and time of day. Some entrances may be locked, and ID badge or intercom access may be required.
