Hospice Care Partner Support

The Hospice Care Partner Support program provides emotional support, practical tools, and a sense of community for individuals caring for someone with a progressive, serious life-limiting illness. Our goal is to help care partners manage the emotional, physical, and social challenges that come with providing care, and to remind them they are not alone in the process.

Who We Help

This program is designed for individuals who:  

  • Are supporting a loved one with a serious or life-limiting illness 
  • Want to connect with others who understand the caregiving experience 
  • Are seeking emotional support, coping tools, and balance while caring for others 

Support is available at any stage of the caregiving journey, including while caring for someone, or as you begin to anticipate the loss of a loved one. 

What to Expect

Our services provide a safe, supportive space to explore your experiences and strengthen your resilience through: 

  • 1:1 Psychotherapy with a licensed Social Worker, offering emotional support for those experiencing anticipatory grief (up to 6 sessions) 
  • Care Partner Support Group (6 sessions) a facilitated group for individuals caring for someone with a life-limiting illness 
  • Education and coping strategies for managing stress, burnout, and emotional fatigue 
  • Opportunities to connect with others who are going through similar experiences 

Groups are small and respectful, ensuring everyone’s voice is heard and valued.

Self-referrals are welcome. 

Healthcare professionals can refer clients directly by emailing hospicereferrals@providencecare.ca or by calling 613-544-4900 ext. 51980.

  • Client is a care partner of an individual living with a life-limiting illness.

Program Information:

Once a referral is received, you will be contacted by the Providence Care Hospice Grief and Bereavement Coordinator to complete an intake discussion and arrange services. 

There are bereavement supports to help you. Speak to a member of the care team to learn more.

Hours of Operation

Our facilities operate 24/7, but building access and available services vary by location and time of day. Some entrances may be locked, and ID badge or intercom access may be required.
