Transitional Care (Inpatient)

Our Inpatient Transitional Care Program at Providence Transitional Care Centre supports individuals who no longer need acute hospital care but aren’t yet ready to return home. The program focuses on your recovery, independence and safe transitions back home or to another care setting.

Who We Help

This program supports individuals aged 16 and older who:

  • Require physical recovery, medication stabilization and have mobility challenges
  • Need short-term support after a hospital stay
  • Aren’t well enough to return home or long-term care
  • Need extra time to regain strength and function
  • May be waiting for home supports or housing to be arranged
  • Are medically stable
