Mobile Response Teams

Mobile Response Teams, part of Providence Cares Seniors Mental Health Behavioural Support Services provide timely assistance to residents in long-term care homes across Southeastern Ontario. They support individuals living with dementia, mental health issues, substance use health, neurological disorders, and associated responsive behaviours.
The teams offer assessments, care planning, and support during transitions to address each resident’s unique needs.

Who We Help

We support people living in long-term care who:

  • Have dementia or memory loss
  • Are living with a mental health or substance use issue
  • Have a neurological condition like Parkinson’s
  • Show behaviours that affect their safety or care

We also work closely with care partners and staff to create safer, more supportive environments.

What to Expect

Behavioural Support

  • Assessment and care planning
  • Identify contributing factors of responsive behaviours
  • Develop personalized strategies that are meaningful and supportive of the individual
  • Model and coach long-term care staff on successful approaches
  • Help integrate these strategies into daily care routines

Transition Support

We support residents during moves:

  • From the community to long-term care
  • Between long-term care homes
  • From long-term care to hospital
  • From hospital back to long-term care

Education and Collaboration

  • Leading care conferences and team meetings
  • Providing resident-specific education
  • Sharing helpful tools and resources
  • Provide best practice education such as Gentle Persuasive Approaches or other

How to Access the Service

Referrals are accepted from long-term care home staff and health-care partners working with individuals who have an open long-term care file.

Adults living in long-term care—or entering care—who are at risk of or experiencing responsive behaviours caused by:

  • Dementia
  • Mental health or substance use disorders
  • Parkinson’s or other neurological conditions
  • Multiple chronic health issues

Hours of Operation

Our facilities operate 24/7, but building access and available services vary by location and time of day. Some entrances may be locked, and ID badge or intercom access may be required.
