Providence Care Hospital
Providence Care Hospital has paused all designated visits during the provincial shutdown. During this time, only essential visitors are permitted to enter the hospital.
All staff, physicians, outpatients, volunteers and visitors are screened at the Main Entrance. Only those who pass screening will be allowed to enter the hospital.
You must answer the following questions:
- Do you have a fever (temperature of ≥ 37.8oC)?
- Do you have any of the following symptoms:
- Cough
- Sore throat
- Shortness of breath
- Chills
- Runny nose, sneezing or nasal congestion (not due to other underlying reasons like seasonal allergies or post-nasal drip)
- Difficulty swallowing
- New and/or unexplained headache
- Unexplained fatigue/malaise
- Unexplained nausea, vomiting, diarrhea or abdominal pain
- Change in your sense of taste/smell
- Have you been tested for COVID-19 due to symptoms and are awaiting results (asymptomatic/surveillance testing does not apply)
- Have you tested positive for COVID-19 in the last 20 days?
- In the last 14 days, have you:
- travelled outside of Ontario?
- travelled outside of Canada?*
- visited**/resided in or had guests from a location in Red or Grey*** zone or outside Ontario?
- In the last 14 days, have you had contact**** with a suspected***** or confirmed case of COVID-19?
*If yes, please note if visitor is an essential worker.
**Visited – spent greater than 15 minutes indoors or within an enclosed space.
***Red and Grey zone as per provincial framework.
****Contact: been advised by Public Health or your employer you have had contact with a
confirmed case, provided care, had similar close physical contact, or had lived with or otherwise
had close, prolonged contact without appropriate personal protective equipment (PPE)
including: gown, gloves, mask and eye protection.
****Asymptomatic or surveillance testing in one of your contacts does not apply.
If you have answered YES to any of the above questions, do not visit.

Providence Manor
General Visitors are not permitted in the Home during the provincial shutdown. During this time, only one (1) designated Essential Caregiver and Essential Visitors (individual who has a loved one who is end-of-life or very ill) will be permitted to enter the Home.
Essential Caregivers and Essential Visitors will be screened upon arrival, this includes having your temperature taken.
All Essential Caregivers must provide proof of a negative COVID-19 test done within the last seven (7) days and subsequently not have tested positive. A screen capture or print out of the online test result is sufficient.
Please answer our screening questions truthfully. If you do not pass screening, your visit will be postponed. You must answer the following questions:
- Do you have a fever (temperature of ≥ 37.8oC)?
- Do you have any of the following symptoms (onset in last seven days?)
- New or worsening cough
- Shortness of breath
- Sore throat
- Chills
- Runny nose, sneezing, nasal congestion (not due to other underlying reasons like seasonal allergies or post-nasal drip)
- Difficulty swallowing
- New and/or unexplained headache
- Unexplained fatigue/malaise
- Unexplained nausea, vomiting, diarrhea or abdominal pain
- Changes in your sense of taste
- Have you been tested for COVID-19 in the last seven days? (Essential Caregivers must provide proof of negative test collected in the last seven days)
- In the last 14 days, have you:
• Returned from travel outside of province or outside of Canada?
• Visited or resided in RED or GREY zone of Ontario?
• Had someone visit or reside with you from a RED or GREY zone OR from out of province? - In the last 14 days, have you had close, unprotected contact* with any of the following:
• a sick traveller
• someone with an infectious respiratory illness
• a probable or confirmed case of COVID-19
*Close, unprotected contact is defined as having provided care, had similar close physical contact, or had lived with or otherwise had close, prolonged contact without appropriate PPE including: gowns, gloves, mask and eye protection.
Asymptomatic or Surveillance testing in one of your close contacts does not apply.

Community Services
Community Services care teams are limiting face-to-face appointments (with exceptions for high-risk individuals), pausing in-person group sessions and connecting with clients/families via telephone and other technologies during the provincial shutdown.
All clients/visitors must answer the following questions before they are allowed to enter a Community Services site:
- Do you have a fever (temperature of ≥ 37.8oC)?
- Do you have any of the following symptoms:
- New or worsening cough
