Designed for health-care providers, community organizations and referral partners, this session will explore practical communication strategies, care partner supports and available referral pathways. This session is also open to care partners who wish to learn more about the program. 

Attendees will hear from clinical experts, program leaders and individuals with lived experience about the impact of the program and how it is helping care partners build confidence, resilience and meaningful connections across Ontario.
