Each wing of the new Providence Care Hospital is named after the location it is closest to on the King Street West site – Heritage, next to the stone, heritage buildings on the site, Lakeview for the connection to the water of Lake Ontario, and Parkside – next to the grassy, natural areas of Lake Ontario Park.
We want you to make your mark on the new hospital by featuring your photography as design elements in each of the wings.
Send us your best nature photos depicting Heritage, Lakeview and Parkside and you could have your picture showcased in the new Providence Care Hospital!
Plus, Providence Care staff members who enter the contest will be entered into the Redevelopment Grand Prize Draw.

The Providence Care Hospital 2015 Photo Contest (the “Contest”) runs from Friday, July 10 at 12:01 am to 11.59 p.m. (EST) November 30, 2015 (the “Contest Period”).
By entering this Contest the entrants and participants automatically agree to accept and abide by these Rules and Regulations.
RULES AND REGULATIONS
1. Eligibility:

To enter and to be eligible to win, entrant must be a legal resident of Canada and be eighteen (18) years of age or older.
2. Photo entries:

Maximum ten (10) photo entries per entrant
Submitted photos must:
-
Be representative of one or more of the following areas:
Heritage (Eg. Limestone buildings, local architecture [greys])
Lakeview (Eg. Eaves, waterfront [blues])
Parkside (Eg. Grass, leaves, trees [greens])

Be high quality (minimum 300dpi) in a digital format (JPEG preferred)

Be the sole and original work of the entrant
-
Not have been significantly modified. Minor digital enhancement is permitted, but photos that have been significantly modified will be disqualified. Not Permitted: borders or frames added to photos; watermarks, signatures, or copyright notices added to photos; adding or replacing elements in a photo

Be suitable for a public audience and adhere both in appearance and in fact to the norms of civil discourse.
3. Entry Period and Method:

All entries must be submitted to info@providencecare.ca; if the file size is too large to email, entries can be uploaded to: https://www.hightail.com/u/ProvidenceCare with a corresponding email to info@providencecare.ca

The subject line for the email entries must state: Providence Care Hospital 2015 Photo Contest – Photo Entry

All entries must include your first and last name, city and providence of residence, and email address.

All entries must be accompanied by a descriptor of each photo that defines when/where the photo was taken and its relation to Heritage, Lakeview or Parkside.

All entries must be received between 12:01 a.m. (EST) July 10, 2015 and 11:59 p.m. (EST), November 30, 2015 in order to be eligible.
4. Determining Winners:

Photographs will be judged based on four (4) criteria: Originality; Creativity; Representativeness; & Quality.
Originality: You must have taken this photo. Is your photo original, fresh or inventive?
Creativity: How imaginative is your photo? A creative photo breaks from traditional methods of presenting ideas and messages.
Representativeness: Is your photo effective in its depiction of the representative areas (Heritage, Lakeview, or Parkside)
Quality: Is your photo a high-quality photo (technically), well composed, and not too dark or light?

After the close of the entry period, winning photos will be chosen from all eligible submissions. Judging will be done by a panel of judges from Providence Care Centre and the community.

Providence Care Centre will advise the winner(s) by telephone or e-mail (using the contact information provided by the entrant at time of entering the contest).
5. Prizes:
-
The winner(s) of the photo contest will have their winning photo(s) showcased in the new hospital (with credit in the name of the photographer). This prize must be accepted as rewarded. There is no cash substitute for the prize.

Winner(s) will be announced on March 5 at an open house at PCH where we will be sharing construction updates and more details about the new hospital.

In addition, entrants who are staff of Providence Care Centre will be provided entered into the Redevelopment Grand Prize Draw (drawn on opening day in spring 2017)
6. Use of Photographs:

By entering the Contest, you grant Providence Care Centre the non-exclusive right to publish, use, distribute, reproduce and publicly display your photograph, and to create derivative works from your photograph, in perpetuity, in any of its online, print or other media publications, materials or exhibitions, for purposes of promoting Providence Care Centre, Providence Care Hospital, and/or its mission and vision, including but not limited to editorial, advertising, publicity, and fundraising purposes, without payment of compensation unless prohibited by law. You grant Providence Care Centre permission to provide your name, and your city and providence of residence, when exhibiting, publishing, reproducing or otherwise using your photo and when identifying you as an entrant or prize winner in the Contest. While Providence Care will make every effort to credit photographers when using photos, there may be circumstances where it is not possible to do so. Subject to the rights granted to Providence Care Centre, you retain all other copyrights and rights to future use of the photographs you enter into the Contest.

You acknowledge and agree that Providence Care Centre may, at its sole discretion and without any communication to you, crop or edit the winning photo(s) as required to meet its space and design needs.

Once a photo submission has been accepted for entry into the contest, the contestant’s name, city and province may be publically identified at the sole discretion of Providence Care Centre. All contestants have the right to request that this information not be publicized in connection with their contest submission. Contestants must inform Providence Care Centre of this wish when submitting photos for the contest.

Releases:
If your photograph(s) include an identifiable person or people, you must secure permission from each person (or in the case of a minor, the minor’s parent or guardian) to use her/his image in the Contest, and for the photograph to be used as set out above. Providence Care Centre reserves the right to disqualify an entry if it finds, in its sole discretion, that an entrant has not secured a necessary release.
7. General Rules:

All decision of Providence Care Centre with respect to any aspect of this Contest, including without limitation the eligibility of entries, are final and binding on all entrants in all matters as they relate to this Contest.

BY ENTERING THIS CONTEST, EACH ENTRANT ABSOLVES PROVIDENCE CARE CENTRE AND ITS RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES AND THEIR AGENTS, REPRESENTATIVES, AND CONTRACTORS FROM ANY AND ALL LIABILITY FOR ANY HARM SHE/HE MAY SUFFER AS A RESULT OF PARTICIPATION IN THE CONTEST, COMPLIANCE OR NON-COMPLIANCE WITH THESE CONTEST RULES, THE AWARDING OF THE PRIZE, OR THE USE OF ANY PERSONAL INFORMATION.

By entering this Contest, the winner authorizes Providence Care Centre, and their respective agents or representatives to use her/his name, city and providence of residence, and/or statements about the Contest and/or the prize for advertising or credit purposes, and to do so without remuneration of any kind.

YOU HEREBY AGREE TO INDEMNIFY, DEFEND AND HOLD HARMLESS PROVIDENCE CARE CENTRE AND ITS RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, REPRESENTATIVES, AND CONTRACTORS FROM AND AGAINST ANY CLAIM, LOSS, OBLIGATION, LIABILITY OR EXPENSE THAT MAY ARISE AS A RESULT OF OR IN CONNECTION WITH YOUR PHOTO(S) SUBMITTED HEREUNDER, OR THE USE OF SUCH PHOTO BY PROVIDENCE CARE CENTRE.

Providence Care Centre is not liable for any entries which are lost, misdirected or late for any reason whatsoever or for: faulty computer components, software or links; the loss or non-existence of communications capability; faulty incomplete, incomprehensible, or erased computer or network transmissions; any problems or technical malfunction of any telephone network or lines, computer on-line systems, servers, access providers, facilities or computer equipment or software; any garbled, lost, late, delayed, destroyed or misdirected e-mail or any computer errors, malfunctions or failure of any e-mail or other electronic transmission to be received or sent by entrants or Providence Care Centre that delay entry, result in disqualification or otherwise render an individual ineligible for this contest; or any combination of the foregoing.

Each photo entry is original to the entrant and that the entrant has all necessary rights in and to the photograph, and that no third party has any right, including, without limitation, moral right, title or interest to the Photo;

You have the full right and authorization to grant the License and to submit the photo to this Contest upon all of the terms and conditions herein stated;

Each photo entry does not violate any law, statute, ordinance or regulation;

Providence Care Centre reserves the right to cancel, terminate, suspend, or modify this Contest, at its sole discretion, including, without limitation, in the event a computer virus or bug, unauthorized human intervention, or any other occurrence beyond its control corrupts or adversely affects the administration, security, impartiality, or intended operation of this Contest.

Any attempt to deliberately damage any website or to undermine the legitimate operation of this Contest may be a violation of criminal and civil laws, and should such an attempt be made, Providence Care Centre reserves the right to seek remedies and damages to the fullest extent permitted by law, including criminal prosecution.

Providence Care Centre reserves the right, in its sole and absolute discretion, to verify that a potential winner is in full compliance with these Contest Rules before the entrant is recognized as a winner, and before awarding any prize.
