Providence Transitional Care Centre (PTCC) will:
- Provide increased capacity to meet the needs of frail, older adults and those living with dementia having responsive behaviours in the South East region.
- Reduce the use and length of stay in regional hospitals and allow us to better respond to urgent priorities such as COVID-19.
- Work closely with primary care partners in the new Ontario Health Team model to provide seniors the care they need. It will enable seniors to age in their community by improving access to timely assessment, care navigation and care delivery.
- Improve health outcomes and quality of life for patients and families.
As part of our commitment to education, research and partnerships, PTCC will be a teaching site for the Faculty of Health Sciences at Queen’s University and for St. Lawrence College programs.
The 64-bed facility will offer specialized inpatient services designed to promote and preserve wellness and functionality in older adults, while addressing gaps in restorative and specialized services.
