The Senior/Geriatric Rehabilitation program at Providence Care Hospital meets the needs of frail seniors with complex health needs that require a multidisciplinary, comprehensive geriatric assessment and may need longer periods of rehabilitation, based on individual tolerance levels for various therapies.
Admission to the program is determined on a case by case basis for patients who meet eligibility criteria. Patients must demonstrate a need and an ability to participate and benefit from regular therapy, including Physiotherapy, Occupational Therapy and/or Speech Language Pathology. Twenty-four hour nursing care is required. Individualized goal based programs are developed, supported by a team of health professionals including geriatricians. Patients must be medically stable and motivated to participate in low to moderate intensity rehabilitation with a focus on enabling patients to achieve transition home or to the next level of care.
